Immunization Records

As a pre-requisite to enrollment, all new students need to document proof of adequate immunization against measles and rubella and show proof of a tuberculosis screening within 12 months prior to registration. An immunization form can be requested from the Admissions Center. This form should be completed prior to your arrival at Lee.
